Use this form to claim tax
back on your donations, complete, print and sign, and return to us at:

sponsorship letter.
Important note: If you return
this form, for every £1 your contribute, One to One will be able to claim
an extra 28p - and you'll get a deduction from your taxable income.
You must pay an amount in income tax and/or Capital Gains tax at least equal
to the tax that the charity reclaims on your doantions in the tax year
(currently 28p for each £1 you give)
You can cancel this declaration at any time by notifying the charity.
Please note Gift Aid can only be claimed on sponsorship where the monies are
not paid by either the participant or "connected persons" to them.
"Connected persons" are: wife or husband; relative (brother, sister,
ancestor - e.g. mother) or lineal descendant - e.g. grandson; wife or
husband of a relative.
If in the future your circumstances change and you no longer pay tax on your
income and Capital Gains equal to the tax that the charity reclaims, you can
cancel your declaration.
If you pay at the higher rate, you can claim further tax relief in your Self
Assessment Tax Return.
If you are unsure whether your donations qualify for Gift Aid tax relief,
ask the charity or ask your local Tax Office for Leaflet IR65.
Please notify the charity if you change your name and address.
The charity can only claim Gift Aid from individuals. If you, as a company,
wish to Gift Aid an amount to the charity, you can claim back tax, but the
charity cannot. In order for the charity to benefit from the Gift Aid
Scheme, you should increase your donation by the amount you will be claiming
back.
